You can get vacuum leaks anywhere in the system that has vacuum, which is basically anything before the cylinder on the intake side (boots as MD86 pointed out, as well as the line to the petcock, the little sync caps at the top of each carb in the same location as the connection from the #1 carb to the petcock, and even the vacuum diaphrams that make up the slides and top of the slides).
The won't run past 4k issue could be either fouled plugs (because of a really bad fuel-air mix, or because of a vacuum leak, or sticky slides), or fuel starvation -- simple way to tell: next time it does it, immediately reach down and turn the petcock to PRIME without stopping. If the symptom disappears almost immediately, it's the petcock vacuum diaphram having failed.
